Hazard identified

The luminaire wiring may overheat if the lightbulbs are too close to or in contact with the wiring, resulting in a possible short-circuit and potential fire.

Some products were not assembled with an approved 2x100 watt medium base cluster. The non-approved cluster may allow the wires to either come into contact with the light bulbs or violate the UL/CSA standard of coming within 2 inches of the bulbs. This can cause overheating of the luminaire wiring.

As of February 2017, the company has received one report of short-circuiting in Canada, and no report of injury.

Health Canada has not received any reports of consumer or complaints related to the use of these products.

Number sold

Approximately 48,000 units of the affected products were sold in Canada.

Time period sold

The affected products were sold between 2003 and 2016 at various retailers and online sellers.

Consumers should immediately stop using the lumniaires and contact DVI Lighting Inc. at customerservice@dvi.lightingor visit dvilighting to identity if your luminaire is affected.

Consumers can consult the website to obtain a QR code or view the Youtube instructional video.

Please note that the Canada Consumer Product Safety Act prohibits recalled products from being redistributed, sold or even given away in Canada.

Health Canada would like to remind Canadians to report any health or safety incidents related to the use of this product or any other consumer product or cosmetic by filling out the Consumer Product Incident Report Form.
