Product description

Wooden Activity Toy with Xylophone, spinning gears and maraca. With Item lot code 217120 IL and UPC code 011964494460.

Hazard identified

The screws attaching the spinning gears may be loose and fall off the toy creating a small parts choking hazard.

As of March 30th 2020, the company has received no reports of incidents in Canada, and no reports of injuries. In the United States, the company has received 5 reports of screws falling off including two reports of children putting the screws in their mouths, and no reports of injuries.

Number sold

Approximately 63 units of the affected product were sold in Canada and approximately 3935 were sold in the United States.

Time period sold

The affected products were sold from October, 2019 to February, 2020.
